  • How to complete the CEU application form.

 

  1. Fill in the contact name and address, phone number, fax number, email, sponsoring organization and title of the program/activity.
     
  2. Check the box indicating whether the program is open to the public to attend, and if so, type in the fee.  In some cases, industry will request CEU's for their in-house training where the public is not invited.
     
  3. Write dates/location of the meeting.
     
  4. In the 'CEU's Requested' section, please indicate the total number of CEU's for each Continuing Education Unit (CEU) area by totaling the CEU's you have listed on page 2.  CEU's are awarded on the basis that one hour of classroom is equal to one CEU.  CEU's are awarded in increments of 0.5 using the following formula:

    30-45 minutes

    = 0.5

    46-75 minutes

    = 1.0

    76-105 minutes

    = 1.5

    106-135 minutes

    = 2.0

    136-165 minutes

    = 2.5

    166-195 minutes

    = 3.0

    196-225 minutes

    = 3.5

    226-255 minutes

    = 4.0


  5. Complete the second page of the application by indicating the date, start and end time of each session, the CEU area(s) to which the session topic relates, the amount of CEU's requested per CEU area, the session topic, a brief description of the session topic and the speaker's name and brief background information for each speaker.
     
    You need to designate the coding for the CEU area column in order to identify the specific training area.  The coding is obtained from the CCA Continuing Education Standards document.  There are 5 CEU categories:  PM (Pest Management), SW (Soil & Water Management), NM (Nutrient Management), CM (Crop Management) and PD (Professional Development).
     
  6. If the speaker is well known in the area, their job title and employer is sufficient background information.  Otherwise, a 1-page resume or bio is suggested.
     
  7. Include a copy of the meeting agenda and/or any supporting information regarding the meeting.
     
  8. You will receive confirmation of CEU's that your program has been approved for.  You will receive CEU sign-up sheets that meeting participants will sign and write their accreditation number.  The sheet is then returned to Mary Jo Rowbottom (608) 273-2081 at the American Society of Agronomy, and CCA's will receive their credits.  If a sign-up sheet is not received prior to the event, collect names using the generic sign-up sheet.
